Form 1 is the request to register a worker with social insurance on hiring, and Form 6 is the notice that his insurance has ended. The National Organization for Social Insurance requires the first within two weeks of the worker joining and the second within a week of his service ending.
Step by step
- 1
Form 1 on hiring
Prepared as an original and two copies or electronically, and submitted to the insurance office covering the business location within two weeks of the worker joining, under Social Insurance and Pensions Law No. 148 of 2019 and its executive regulations.
- 2
Form 1 attachments
A birth date document, the national ID card, the qualification, the appointment decision or contract, a medical fitness report on Form 111, Form 3 if there are earlier contribution periods, Form 4 stating any earlier periods or that no other pension is due, and Form 5 for insurance coverage data.
- 3
Form 6 at the end of service
Submitted to the fund office where the employer is registered within a week of the service ending, with the end date and reason and declarations by the insured worker and the responsible manager.
- 4
If the worker will not sign
The form the Organization provides has a section for disputes, where the responsible manager confirms sending a copy of the notice to the worker by registered letter with acknowledgment of receipt.
- 5
Keep the proof
Keep a copy of each form or proof of electronic submission in the employee file, because Article 92 of Labor Law No. 14 of 2025 makes proof of insurance part of the file.
Having an employee sign a blank Form 6 at hiring is a common and wrong practice. The form reports the fact, date and reason of the end of service, and a resignation is valid only under Article 167 of the Labor Law.
